Voisin\/WP)<\/strong><\/p>\n\n\n\n<p><strong>05 february 24<\/strong>  (RSN.org)<\/p>\n\n\n\n<blockquote class=\"wp-block-quote\">\n<p><strong><em>The right-wing policy agenda written for a new Donald Trump presidency would \u201cgreatly accelerate\u201d efforts to privatize Medicare<\/em><\/strong><\/p>\n<\/blockquote>\n\n\n\n<p>Last year, for the first time ever, a majority of Americans eligible for&nbsp;<a target=\"_blank\" href=\"https:\/\/www.rollingstone.com\/t\/medicare\/\" rel=\"noreferrer noopener\">Medicare<\/a>&nbsp;were on privatized Medicare Advantage plans. If Republicans win the presidential race this year, the push to fully privatize Medicare, the government health insurance program for seniors and people with disabilities, will only intensify.<\/p>\n\n\n\n<p>Conservative operatives have already sketched out what the GOP\u2019s policy agenda would look like in the early days of a new&nbsp;<a target=\"_blank\" href=\"https:\/\/www.rollingstone.com\/t\/donald-trump\/\" rel=\"noreferrer noopener\">Donald Trump<\/a>&nbsp;presidency. As&nbsp;<em>Rolling Stone<\/em>&nbsp;has detailed, the proposed Project 2025 agenda is&nbsp;<a target=\"_blank\" href=\"https:\/\/www.rollingstone.com\/politics\/politics-features\/maga-plan-attack-birth-control-surveil-women-ban-abortion-pill-1234934807\/\" rel=\"noreferrer noopener\">radically right-wing<\/a>. One item buried in the 887-page&nbsp;<a target=\"_blank\" href=\"https:\/\/thf_media.s3.amazonaws.com\/project2025\/2025_MandateForLeadership_FULL.pdf\" rel=\"noreferrer noopener\">blueprint<\/a>&nbsp;has attracted little attention thus far, but would have a monumental impact on the health of America\u2019s seniors and the future of one of America\u2019s most popular social programs: a call to \u201cmake Medicare Advantage the default enrollment option\u201d for people who are newly eligible for Medicare.<\/p>\n\n\n\n<p>Such a policy would hasten the end of the traditional Medicare program, as well as its foundational premise: that seniors can go to any doctor or provider they choose. The change would be a boon for private health insurers \u2014 which generate&nbsp;<a target=\"_blank\" href=\"https:\/\/www.healthcaredive.com\/news\/medicare-advantage-profitability-decline-moodys\/706000\/\" rel=\"noreferrer noopener\">massive profits<\/a>&nbsp;and&nbsp;<a target=\"_blank\" href=\"https:\/\/wendellpotter.substack.com\/p\/first-half-of-2023-7-big-health-insurers\" rel=\"noreferrer noopener\">growing portions of their revenues<\/a>&nbsp;from Medicare Advantage plans \u2014 and further consolidate corporate control over the United States&nbsp;<a target=\"_blank\" href=\"https:\/\/www.rollingstone.com\/t\/health-care\/\" rel=\"noreferrer noopener\">health care<\/a>&nbsp;system.<\/p>\n\n\n\n<p>It would not likely benefit seniors, since the private plans limit the doctors they can see and often wrongfully deny patients\u2019 care. Because the plans are costly, experts say the GOP proposal could threaten the Medicare program\u2019s solvency.<\/p>\n\n\n\n<p>Philip Verhoef, president of the single-payer advocacy group Physicians for a National Health Program, tells&nbsp;<em>Rolling Stone<\/em>&nbsp;it would be \u201cdisastrous\u201d to make Medicare Advantage the default enrollment option. \u201cTo do so would be really just a clear handout to the private insurance industry,\u201d he says.<\/p>\n\n\n\n<p>David Lipschutz, associate director of the Center for Medicare Advocacy, says the plan would \u201cgreatly accelerate\u201d Medicare privatization.<\/p>\n\n\n\n<p>Project 2025, which is being spearheaded by the influential&nbsp;<a target=\"_blank\" href=\"https:\/\/www.rollingstone.com\/t\/heritage-foundation\/\" rel=\"noreferrer noopener\">Heritage Foundation<\/a>, describes its presidential transition project as \u201cthe conservative movement\u2019s unified effort to be ready for the next conservative Administration to govern at 12:00 noon, January 20, 2025.\u201d<\/p>\n\n\n\n<p>Established in 1973, the Heritage Foundation has long exerted influence in GOP administrations \u2014 and perhaps never more so than in the one led by Trump. One year into his presidency, the Heritage Foundation wrote that \u201cTrump and his administration have embraced nearly two-thirds of the policy recommendations\u201d in its \u201cMandate for Leadership.\u201d<\/p>\n\n\n\n<p>The latest \u201cMandate for Leadership,\u201d the Project 2025&nbsp;<a target=\"_blank\" href=\"https:\/\/thf_media.s3.amazonaws.com\/project2025\/2025_MandateForLeadership_FULL.pdf\" rel=\"noreferrer noopener\">agenda<\/a>&nbsp;written by the Heritage Foundation and like-minded conservative organizations, would reshape the future of the Medicare program and push more seniors into alternative, privatized Medicare Advantage plans.<\/p>\n\n\n\n<p>The trend toward privatization is already happening \u2014 a&nbsp;<a target=\"_blank\" href=\"https:\/\/www.kff.org\/policy-watch\/half-of-all-eligible-medicare-beneficiaries-are-now-enrolled-in-private-medicare-advantage-plans\/\" rel=\"noreferrer noopener\">slim majority<\/a>&nbsp;of Medicare beneficiaries is now enrolled in private Medicare Advantage plans. Trump helped advance this trend as president.<\/p>\n\n\n\n<p>As&nbsp;<em>The New York Times<\/em>&nbsp;<a target=\"_blank\" href=\"https:\/\/www.nytimes.com\/2018\/12\/01\/us\/politics\/trump-medicare-advantage-plans.html\" rel=\"noreferrer noopener\">reported<\/a>&nbsp;in 2018, during Medicare\u2019s open enrollment period, the Trump administration emailed messages to millions of beneficiaries touting the private plans. Some of the emails included subject lines like, \u201cGet more benefits for your money,\u201d and \u201cSee if you can save money with Medicare Advantage.\u201d<\/p>\n\n\n\n<p>Trump\u2019s administration also helped make Medicare Advantage more attractive by&nbsp;<a target=\"_blank\" href=\"https:\/\/www.bloomberg.com\/news\/articles\/2019-10-09\/medicare-advantage-plans-2020-cigna-aetna-offer-competing-plans\" rel=\"noreferrer noopener\">expanding<\/a>&nbsp;the range of perks the plans can offer to enrollees, allowing them to add benefits such as transportation to doctors\u2019 offices and meal delivery.<\/p>\n\n\n\n<p>A spokesperson for Trump did not respond to questions from&nbsp;<em>Rolling Stone<\/em>&nbsp;about whether the former president supports the Project 2025 proposal.<\/p>\n\n\n\n<p>Former South Carolina Gov. Nikki Haley, Trump\u2019s sole remaining GOP primary opponent, has&nbsp;<a target=\"_blank\" href=\"https:\/\/www.rev.com\/blog\/transcripts\/desantis-vs-haley-cnn-gop-debate-transcript\" rel=\"noreferrer noopener\">said<\/a>&nbsp;she wants to \u201cexpand Medicare Advantage plans, which seniors love.\u201d<\/p>\n\n\n\n<p>Lipschutz, of the Center for Medicare Advocacy, says the Project 2025 plan would \u201cviolate the free choice of provider provision and principle of the Medicare Act, which guarantees that people will have a free choice of provider.\u201d<\/p>\n\n\n\n<p>Under traditional Medicare, enrollees are free to visit any doctor who accepts Medicare (<a target=\"_blank\" href=\"https:\/\/www.kff.org\/medicare\/issue-brief\/how-many-physicians-have-opted-out-of-the-medicare-program\/\" rel=\"noreferrer noopener\">nearly all<\/a>&nbsp;physicians do). Medicare Advantage plans, by contrast, often have limited networks of doctors that patients can choose from, and many providers&nbsp;<a target=\"_blank\" href=\"https:\/\/www.beckershospitalreview.com\/finance\/hospitals-are-dropping-medicare-advantage-left-and-right.html\" rel=\"noreferrer noopener\">have stopped accepting<\/a>&nbsp;the private plans because they so often deny the prior authorization requests they require before patients can receive services.<\/p>\n\n\n\n<p>Lipschutz notes the Project 2025 plan says that Republicans would \u201cgive beneficiaries direct control of how they spend Medicare dollars.\u201d He says this is \u201cinternally inconsistent\u201d with its proposal to make Medicare Advantage the default enrollment option. Medicare Advantage plans, he says, are \u201ctasked with managing your care, and telling you what you can and can\u2019t do, and what is and is not covered \u2014 that is the opposite of putting beneficiaries in control of how they spend their dollars.\u201d<\/p>\n\n\n\n<p>It\u2019s natural that beneficiaries might see some appeal in Medicare Advantage plans. Traditional Medicare leaves patients with significant out-of-pocket costs \u2014 which is why many enrollees in the traditional program pay for supplemental Medigap coverage. Medicare Advantage plans also promise to limit these costs. And unlike the traditional Medicare program, the private plans can offer dental, vision, and hearing benefits.<\/p>\n\n\n\n<p>But the private plans often do not work well for sicker patients \u2014 and when they seek to&nbsp;<a target=\"_blank\" href=\"https:\/\/news.bloomberglaw.com\/health-law-and-business\/sicker-patients-leave-medicare-advantage-for-traditional-medicare\" rel=\"noreferrer noopener\">leave the program<\/a>&nbsp;and enroll in traditional Medicare, they&nbsp;<a target=\"_blank\" href=\"https:\/\/fortune.com\/well\/2024\/01\/06\/older-americans-say-they-feel-trapped-in-medicare-advantage-plans\/\" rel=\"noreferrer noopener\">may encounter<\/a>&nbsp;an expensive problem: Most states allow Medigap plans to deny coverage to patients with preexisting health conditions, so these patients can get stuck with significant out-of-pocket costs.<\/p>\n\n\n\n<p>While Medicare Advantage plans are required to pay for services covered by traditional Medicare, the government has for years&nbsp;<a target=\"_blank\" href=\"https:\/\/oig.hhs.gov\/oei\/reports\/oei-09-16-00410.pdf\" rel=\"noreferrer noopener\">flagged<\/a>&nbsp;\u201c persistent problems related to denials of care and payment in Medicare Advantage.\u201d More recently, news reports have found the private plans are using&nbsp;<a target=\"_blank\" href=\"https:\/\/www.statnews.com\/2023\/03\/13\/medicare-advantage-plans-denial-artificial-intelligence\/\" rel=\"noreferrer noopener\">artificial intelligence<\/a>&nbsp;and&nbsp;<a target=\"_blank\" href=\"https:\/\/www.statnews.com\/2023\/11\/14\/unitedhealth-algorithm-medicare-advantage-investigation\/\" rel=\"noreferrer noopener\">algorithms<\/a>&nbsp;to deny services in bulk, with extreme&nbsp;<a target=\"_blank\" href=\"https:\/\/www.fiercehealthcare.com\/payers\/unitedhealthcare-hit-class-action-over-alleged-use-ai-deny-medicare-advantage-claims\" rel=\"noreferrer noopener\">error rates<\/a>&nbsp;\u2014 allegedly as high as 90 percent, according to one class action lawsuit.<\/p>\n\n\n\n<p>The Biden administration&nbsp;<a target=\"_blank\" href=\"https:\/\/www.hhs.gov\/about\/news\/2023\/04\/05\/hhs-finalizes-rule-strengthen-medicare-improve-access-affordable-prescription-drug-coverage-hold-private-insurance-companies-accountable.html\" rel=\"noreferrer noopener\">has issued<\/a>&nbsp;<a target=\"_blank\" href=\"https:\/\/www.theguardian.com\/us-news\/2024\/feb\/02\/new-federal-rule-insurance-authorization-surgery-medication\" rel=\"noreferrer noopener\">new rules<\/a>&nbsp;and guidance to try to ensure that Medicare Advantage plans do as they are required and approve the same services as traditional Medicare. But&nbsp;<a target=\"_blank\" href=\"https:\/\/oig.hhs.gov\/oei\/reports\/OEI-09-18-00260.pdf\" rel=\"noreferrer noopener\">the financial incentive to deny care<\/a>&nbsp;is baked into the Medicare Advantage model: The private plans are given a fixed amount of money every month to provide coverage for each enrollee; paying out fewer dollars means extra profit.<\/p>\n\n\n\n<p>Lipschutz says making Medicare Advantage the default enrollment option would \u201caccelerate program insolvency because Medicare Advantage [costs] so much more than traditional Medicare.\u201d<\/p>\n\n\n\n<p>The privatized plans have, for years,&nbsp;<a target=\"_blank\" href=\"https:\/\/www.levernews.com\/the-20-billion-scam-at-the-heart-of-medicare-advantage\/\" rel=\"noreferrer noopener\">systematically<\/a>&nbsp;<a target=\"_blank\" href=\"https:\/\/www.levernews.com\/insurers-are-gaming-medicare-to-the-tune-of-140_billion\/\" rel=\"noreferrer noopener\">overbilled<\/a>&nbsp;the government \u2014 mostly by billing the government as if their patients are sicker than they really are. The Biden administration has worked to&nbsp;<a target=\"_blank\" href=\"https:\/\/www.nytimes.com\/2023\/03\/31\/health\/medicare-overbilling-insurance.html\" rel=\"noreferrer noopener\">claw back some<\/a>&nbsp;of those past overpayments and rein in future outlays. Industry lobbyists and lawyers have aggressively fought these efforts.<\/p>\n\n\n\n<p>MedPAC, an independent congressional advisory agency,&nbsp;<a target=\"_blank\" href=\"https:\/\/www.medpac.gov\/wp-content\/uploads\/2023\/10\/MedPAC-MA-status-report-Jan-2024.pdf\" rel=\"noreferrer noopener\">projects<\/a>&nbsp;that the federal government will overpay Medicare Advantage plans by&nbsp;<a target=\"_blank\" href=\"https:\/\/www.healthcaredive.com\/news\/medicare-advantage-overpayments-medpac-2024\/704579\/\" rel=\"noreferrer noopener\">$88 billion<\/a>&nbsp;this year.<\/p>\n\n\n\n<p>Lipschutz notes the Project 2025 plan includes a few Medicare Advantage reform ideas that could save the government money, depending on the details.
