It\u2019s high time the US did better.\u201d<\/h2>\n\n\n\n<p><a href=\"https:\/\/www.commondreams.org\/author\/julia-conley\">Julia Conley<\/a><\/p>\n\n\n\n<p>May 28, 2026 (CommonDreams.org)<\/p>\n\n\n\n<p>An annual analysis that examines&nbsp;<a href=\"https:\/\/www.commondreams.org\/tag\/healthcare\">healthcare<\/a>&nbsp;systems across nearly two dozen wealthy countries around the world once again highlighted the&nbsp;<a href=\"https:\/\/www.commondreams.org\/tag\/united-states\">United States<\/a>\u2019 \u201cuniquely poor performance relative to its peers,\u201d with this year\u2019s&nbsp;<a href=\"https:\/\/www.commonwealthfund.org\/publications\/issue-briefs\/2026\/may\/us-health-care-global-perspective-2026\"><em>US&nbsp;<\/em><\/a><a href=\"https:\/\/www.commonwealthfund.org\/publications\/issue-briefs\/2026\/may\/us-health-care-global-perspective-2026\"><em>Health Care from a Global Perspective<\/em><\/a>&nbsp;report focusing on \u201cinsurance coverage and access to care, affordability of care, delivery of care, and equity of health outcomes.\u201d<\/p>\n\n\n\n<p>As advocates for expanding the US&nbsp;<a href=\"https:\/\/www.commondreams.org\/tag\/medicare\">Medicare<\/a>&nbsp;system to the entire population have long warned, the country\u2019s for-profit healthcare system\u2014which ties the ability to get care to one\u2019s employment and allows insurance companies to boost profits by&nbsp;<a href=\"https:\/\/www.commondreams.org\/news\/medicare-advantage-ai\" target=\"_blank\" rel=\"noreferrer noopener\">denying<\/a>&nbsp;care to&nbsp;<a href=\"https:\/\/www.commondreams.org\/news\/united-healthcare-surgery-coverage\" target=\"_blank\" rel=\"noreferrer noopener\">patients<\/a>\u2014\u201cThe US, on average, has the poorest health outcomes of any high-income country,\u201d the Commonwealth Fund\u2019s report reads.<\/p>\n\n\n\n<p>RECOMMENDED&#8230;<\/p>\n\n\n\n<figure class=\"wp-block-image\"><a href=\"https:\/\/www.commondreams.org\/news\/gop-medicaid-cuts-hospital-closures\"><img decoding=\"async\" src=\"https:\/\/www.commondreams.org\/media-library\/remote-medical-clinic.jpg?id=66686413&amp;width=1200&amp;height=1200&amp;quality=70&amp;coordinates=833%2C0%2C834%2C0\" alt=\"Remote medical clinic\"\/><\/a><\/figure>\n\n\n\n<h2 class=\"wp-block-heading\"><a href=\"https:\/\/www.commondreams.org\/news\/gop-medicaid-cuts-hospital-closures\">900 Health Facilities Shutting Down or at Risk of Collapse as Trump-GOP Cuts \u2018Ripple Across the Country\u2019<\/a><\/h2>\n\n\n\n<figure class=\"wp-block-image\"><a href=\"https:\/\/www.commondreams.org\/news\/us-k-shaped-economy-graphic\"><img decoding=\"async\" src=\"https:\/\/www.commondreams.org\/media-library\/us-stocks-markets-nyse.jpg?id=56521742&amp;width=1200&amp;height=1200&amp;quality=70&amp;coordinates=959%2C0%2C959%2C0\" alt=\"US-STOCKS-MARKETS-NYSE\"\/><\/a><\/figure>\n\n\n\n<h2 class=\"wp-block-heading\"><a href=\"https:\/\/www.commondreams.org\/news\/us-k-shaped-economy-graphic\">\u2018Biggest Wealth Divide in Modern History\u2019: Graphic Shows Shocking Reality of US Economy<\/a><\/h2>\n\n\n\n<p>The report examines the US system compared with other countries in the Organization for Economic Cooperation and Development (OECD), including the&nbsp;<a href=\"https:\/\/www.commondreams.org\/tag\/united-kingdom\">United Kingdom<\/a>,&nbsp;<a href=\"https:\/\/www.commondreams.org\/tag\/france\">France<\/a>,&nbsp;<a href=\"https:\/\/www.commondreams.org\/tag\/new-zealand\">New Zealand<\/a>, Japan, and Mexico.<\/p>\n\n\n\n<p>It finds that the US spent 18% of its gross domestic product on healthcare in 2024\u2014nearly twice the OECD average.<\/p>\n\n\n\n<p>Life expectancy in the US reached an all-time high in 2024, but was still among the shortest when compared to the 19 other countries, nearly five years shorter than Japan,&nbsp;<a href=\"https:\/\/www.commondreams.org\/tag\/spain\">Spain<\/a>, and&nbsp;<a href=\"https:\/\/www.commondreams.org\/tag\/switzerland\">Switzerland<\/a>, and longer than the average lifespan in&nbsp;<a href=\"https:\/\/www.commondreams.org\/tag\/turkey\">Turkey<\/a>&nbsp;and Mexico.<\/p>\n\n\n\n<p>While the US and Mexico also both rank high on the list in terms of preventable deaths, the latter nation announced last month that it would soon be joining every other country included in the analysis by&nbsp;<a href=\"https:\/\/www.commondreams.org\/news\/wendell-potter-mexico\" target=\"_blank\" rel=\"noreferrer noopener\">shifting<\/a>&nbsp;to a universal, government-run healthcare system.<\/p>\n\n\n\n<p>In the United States, the for-profit health sector\u2014which spent a record&nbsp;<a href=\"https:\/\/www.opensecrets.org\/industries\/lobbying?cycle=2024&amp;ind=H\" target=\"_blank\" rel=\"noreferrer noopener\">$877.69 million<\/a>&nbsp;on lobbying last year\u2014contributes to the high number of avoidable deaths, which stands at 312 per 100,000 people. About 27 million Americans are still uninsured, more than 16 years after the passage of the&nbsp;<a href=\"https:\/\/www.commondreams.org\/tag\/affordable-care-act\">Affordable Care Act<\/a>, and the Republican Party\u2019s refusal to continue ACA subsidies last year as well as its $1 trillion in&nbsp;<a href=\"https:\/\/www.commondreams.org\/tag\/medicaid\">Medicaid<\/a>&nbsp;cuts over the next decade, according to Thursday\u2019s report, are \u201cprojected to increase the number of uninsured Americans by an additional 17 million by 2034, potentially leading to more than 50,000 additional preventable deaths annually.\u201d<\/p>\n\n\n\n<p>\u201cBy contrast, Mexico\u2019s recently established Universal Health Service will provide all residents with access to free care at any&nbsp;<a href=\"https:\/\/www.commondreams.org\/tag\/public-health\">public health<\/a>&nbsp;institute, starting in 2027,\u201d the report states. \u201cThe US is one of the only countries to have enacted policies that&nbsp;<em>reduce<\/em>&nbsp;coverage.\u201d<\/p>\n\n\n\n<p>High out-of-pocket costs may also contribute to poor outcomes and the high number of preventable deaths in the US, the Commonwealth Fund suggests. Americans spend $400 per person, per year, on out-of-pocket costs for prescription drugs, while people in France spend $100.<\/p>\n\n\n\n<p>\u201cThe US is one of the only countries to have enacted policies that&nbsp;<em>reduce<\/em>&nbsp;coverage.\u201d<\/p>\n\n\n\n<p>\u201cIn the US, where approximately 8% of the population is uninsured and one-quarter has coverage that comes with high out-of-pocket costs or deductibles, people are far more likely to forgo needed care because of costs than people in peer countries,\u201d reads the report. \u201c<a href=\"https:\/\/www.commonwealthfund.org\/publications\/issue-briefs\/2026\/may\/us-health-care-global-perspective-2026#footnote12_XkPnbbWr7yOd7jwLkr5tFf66cXmMS-KjFzjeEsZ2UM_wGXzh7CdCBDV\" target=\"_blank\" rel=\"noreferrer noopener\"><\/a>This can mean not filling prescriptions, not obtaining diagnostic tests, treatment, or follow-up care, or being unable to adhere to clinician-recommended care plans.\u201d<\/p>\n\n\n\n<p>The report also identifies the US as a country that lags behind its peers in producing new doctors, contributing to a crisis in primary care, with the US having the fewest number of primary care providers per 1,000 people. The country also has the \u201chighest medical tuition fees of any country in our analysis,\u201d said the Commonwealth Fund.<\/p>\n\n\n\n<p>The organization also found that in 2023, the US had nearly 19 maternal deaths for every 100,000 live births, representing a decline for the country that has long had \u201camong the highest rates of maternal deaths related to complications of pregnancy and childbirth.\u201d<\/p>\n\n\n\n<p>\u201cBy contrast, in 11 of the 18 countries we studied there were less than five maternal deaths per 100,000 live births,\u201d reads the report, which also notes that in the US, maternal mortality is \u201cexceptionally high\u201d among Black women, at 50 deaths per 100,000 live births.<\/p>\n\n\n\n<p>\u201cThis far exceeds national maternal mortality in any of the other countries,\u201d the report states. \u201cInequities in access to care and patients\u2019 care experiences\u2014often rooted in discrimination and clinician bias\u2014may be prime contributing factors.\u201d<\/p>\n\n\n\n<p>Dr. Joseph Betancourt, president of the Commonwealth Fund,&nbsp;<a href=\"https:\/\/www.commonwealthfund.org\/sites\/default\/files\/2026-05\/Gunja_us_health_care_global_perspective_2026_PR.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">noted<\/a>&nbsp;that \u201cthe US has long prided itself on having the best healthcare in the world, but the population benefits from this excellence unevenly, and it remains largely out of reach for many Americans.\u201d<\/p>\n\n\n\n<p>\u201cWe spend more than any other nation on healthcare, so our poorer health outcomes aren\u2019t due to a lack of resources\u2014it is about how we choose to use them,\u201d said Betancourt. \u201cWe know what high-performing health systems look like\u2014other countries have them and are building them. It\u2019s high time the US did better.\u201d<\/p>\n\n\n\n<p>\u201cOther countries have shown that alternatives work. What\u2019s striking isn\u2019t the absence of solutions; it\u2019s our reluctance to implement them.\u201d<\/p>\n\n\n\n<p>The report does not explicitly call on the US to shift to a universal, government-funded healthcare system, but studies have shown that expanding Medicare to the entire US population, as lawmakers including Sen.&nbsp;<a href=\"https:\/\/www.commondreams.org\/tag\/bernie-sanders\">Bernie Sanders<\/a>&nbsp;(I-Vt.) and Reps.&nbsp;<a href=\"https:\/\/www.commondreams.org\/tag\/pramila-jayapal\">Pramila Jayapal<\/a>&nbsp;(D-Wash.) and&nbsp;<a href=\"https:\/\/www.commondreams.org\/tag\/alexandria-ocasio-cortez\">Alexandria Ocasio-Cortez<\/a>&nbsp;(D-NY) have consistently demanded, would address many of the problems listed in the report.<\/p>\n\n\n\n<p>Studies by the Congressional&nbsp;<a href=\"https:\/\/www.commondreams.org\/tag\/budget\">Budget<\/a>&nbsp;Office and Yale University have shown that&nbsp;<a href=\"https:\/\/www.commondreams.org\/tag\/medicare-for-all\">Medicare for All<\/a>&nbsp;would save an estimated&nbsp;<a href=\"https:\/\/www.commondreams.org\/news\/2020\/12\/11\/seems-good-policy-cbo-shows-medicare-all-could-cover-everyone-650-billion-less-year\" target=\"_blank\" rel=\"noreferrer noopener\">$650 billion<\/a>&nbsp;and&nbsp;<a href=\"https:\/\/www.commondreams.org\/news\/medicare-for-all-reintroduction-2025\" target=\"_blank\" rel=\"noreferrer noopener\">prevent 68,000 avoidable deaths<\/a>&nbsp;each year.<\/p>\n\n\n\n<p>The policy, which has been proposed in Congress numerous times, is also broadly popular; 65% of US voters\u2014including 78% of Democrats, 71% of Independents, and 49% of Republicans\u2014support creating a national, government-run healthcare program,&nbsp;<a href=\"https:\/\/www.commondreams.org\/news\/medicare-for-all-popular\" target=\"_blank\" rel=\"noreferrer noopener\">according<\/a>&nbsp;to a Data for Progress poll last year.<\/p>\n\n\n\n<p>Despite this, both Republican and Democratic lawmakers continue to insist the proposal is unpopular and too expensive, with Michigan state Sen. Mallory McMorrow (D-8), who is running against vehement Medicare for All advocate Abdul El-Sayed in the Democratic&nbsp;<a href=\"https:\/\/www.commondreams.org\/tag\/us-senate\">US Senate<\/a>&nbsp;primary,&nbsp;<a href=\"https:\/\/x.com\/KweenInYellow\/status\/2056697990262169630\" target=\"_blank\" rel=\"noreferrer noopener\">insisting<\/a>&nbsp;recently that \u201cthe support for a true&nbsp;<a href=\"https:\/\/www.commondreams.org\/tag\/single-payer\">single-payer<\/a>&nbsp;system isn\u2019t there yet.\u201d<\/p>\n\n\n\n<p>Reginald Williams II, senior vice president at Commonwealth Fund, emphasized that it is \u201cnot inevitable\u201d that \u201cAmericans pay more for healthcare and get less in return.\u201d<\/p>\n\n\n\n<p>\u201cIt\u2019s the result of different choices,\u201d he said. \u201cOther countries have shown that alternatives work. What\u2019s striking isn\u2019t the absence of solutions; it\u2019s our reluctance to implement them. The failure of the US health system is not a failure of ideas.
